DODD APPLICATION
This application is a requirement of the DODD and must be provided prior to class. Completed applications can be emailed to jSchafer@nursingnavigators.org or fax to 330-838-2243. ​ Page 2 of the application will be provided and completed in class. *By signing page one, you confirm that all of the state required checks have been completed, results obtained, and attendee is eligible to take the medication course
INDEPENDENT PROVIDER REQUIREMENTS
Independent Providers MUST have a DODD provider number prior to being certified. Prior to class the following must be submitted to Jes Schafer at JSchafer@nursingnavigators.org Evidence of High School Diploma or GED A copy of a state Issued ID Page 1 of the DODD application (page 2 of the application will be completed in class)​
ADDITIONAL CERTIFICATES
To earn or renew Category 2 or 3 certificates, you must have a valid Category 1 Certification in good standing
RENEWAL DATES
Your DODD certifications need to be renewed every year! Make sure to complete your renewal by the expiration date to keep current. If you happen to miss it, don't worry! You have 60 days to renew, but remember, you'll need to be recertified before jumping back into administering or performing any tasks covered by your certificates. REFUND POLICY
Refunds are not offered after purchase. This policy is in place to ensure that all sales are final, which helps maintain the integrity of our business practices.
